The assessment tools developed by the Feeding Flock are available for your non-commercial use in clinical practice, education, and research. You must agree to the following terms to download the assessment tools.
These assessment tools are protected by copyright. Your right to use of the assessment tools is limited to the right to use the tools for your personal, non-commercial use in accordance with the terms and conditions below.
I acknowledge and agree that I may not reproduce, publish, share, distribute, or sell any of the assessment tools to anyone without prior written approval from a member of the Feeding Flock and an appropriate license agreement executed by the author(s) of the tools.
I agree that I will reference the tools by their correct names and credit the specific authors of the tools in my own presentations, publications, or any other work that I may generate using the tools. Reference information is available on the front page of each assessment tool.
I acknowledge and agree that I may not alter the assessment tools in any way or create any derivative work from any of the assessment tools, including translating the tools into other languages, without the prior written approval from a member of the Feeding Flock.
I agree to use the assessment tools solely as intended (as set forth on the front page of each assessment tool).
I acknowledge and agree that these assessment tools are not diagnostic tools and do not replace a healthcare provider’s assessment.
I agree that the assessment tools are being provided “as is” and that neither the Feeding Flock nor any of the authors of the tools make any warranty with respect to the tools.
I acknowledge and agree that neither the Feeding Flock nor any of the authors of the tools or their institutional affiliations shall have any liability with respect to the tools or the use of the tools, including ANY DIRECT, INDIRECT, SPECIAL, INCIDENTAL, PUNITIVE, CONSEQUENTIAL, OR OTHER DAMAGES.
I hereby waive and agree to release the Feeding Flock, the authors of the tools, and their institutional affiliations from and against all claims, liabilities, and damages of any kind arising out of my use of the tools.
